General Rules for the I-90

If you are a Lawful Permanent Resident, you may use this form to apply for a replacement or renewal of your existing Permanent Resident Card in the following cases:

If your card will be expiring in the next 6 months or if your card has already expired (Renewal).
Your card was lost, stolen, destroyed or partially destroyed.
Your existing card has incorrect data because of a USCIS error.
Your name or other biographic information has been legally changed since the issuance of your existing card.
